Using Insurance
I am not an in-network provider with any insurance companies. I know this is discouraging and I understand. Insurance companies make it very difficult for sole practitioners to conduct business in a timely and effective manner and many, not all, do not offer coverage for nutrition therapy unless you have metabolic disease or diabetes. If insurance or finances are a barrier to getting care, please reach out to discuss.
For those with benefits, please verify your out-of-network benefits prior to your visit. Inquire about CPT codes 97802, 97803, and 97804. Some insurance plans only cover nutrition counseling for specific problems and rarely for prevention or early intervention unfortunately. Confirm any limitations regarding the number of visits, diagnosis type, and telemedicine.
If you wish to use insurance, you must obtain a diagnosis from your medical provider for which medical nutrition therapy is indicated otherwise you will be unable to submit a claim (a script, phone call or copy of your last chart note from the provider is perfect- no formal referrals are needed). Upload it to your profile or request to have it faxed to 1-833-780-1656.

Self-submission: Insurance companies provide instructions and claim forms online for their members. Upon request, I will provide you with a receipt that you will submit to your insurance company with your claim form for direct reimbursement.

I may be able to submit claims on your behalf and charge only co-payments or co-insurance pending insurance reimbursement. Please call to discuss this option if it helps you get care.
Princeton University Students: To support students with getting their healthcare needs met, I will submit claims through the SHP on your behalf. Please verify your benefits before scheduling. You will not have to make any payments at the time of service as long as you participate in the Student Health Plan.
